Medication Administration

We administer prescribed medications to students with completed forms from their doctor and a signed parental consent form. Medication is kept safe in our custom locking medication cabinets.

The Physician Instructions form must be completed by you child's physician and the Parental Consent form must be completed by the child's parent or guardian. Once the forms are completed, the medication and forms need to be brought into the office by a parent or guardian. Please do not send medications with students.

If your students carries any medication on them, we will need these documents filled out and they must specify that the student can self-carry the medication prescribed. Students are unable to carry medications at school if we do not have the proper forms completed.
